Rhinoplasty

Rhinoplasty is a highly specialized surgical procedure that aims to alter the shape, size and proportions of the nose, improving its aesthetic appearance and, in many cases, also its respiratory function. It is considered one of the most challenging plastic surgeries because it requires a deep understanding of the three-dimensional nasal anatomy and high technical precision.

Relevant nasal anatomy:

The nose is composed of:

  • Bone structure (bony nasal pyramid) - upper third

  • Upper lateral cartilages - middle third

  • Alar cartilages (major and minor) - lower third

  • Nasal septum (cartilaginous and bony) - central internal structure

  • Internal and external nasal valves - essential for breathing

  • Skin and subcutaneous tissue - external covering of variable thickness

Surgical approaches:

Closed (endonasal) rhinoplasty: All incisions are made inside the nostrils, with no visible external scars. Provides limited visualization but less postoperative edema.

Open rhinoplasty: Includes a small incision in the columella (structure between the nostrils), providing complete exposure of the nasal structure. Allows for greater precision in complex cases.

Ultrasonic rhinoplasty: Uses ultrasonic equipment to sculpt the nasal bones with greater precision and less trauma.

Preservative rhinoplasty: A modern approach that keeps the nasal dorsum intact while modifying underlying structures.

Structural rhinoplasty: Focuses on strengthening and reconstructing the nasal supporting structures for long-lasting results.

Functional aspects:

Modern rhinoplasty emphasizes the preservation or improvement of respiratory function through:

  • Correction of septal deviation

  • Treatment of nasal concha hypertrophy

  • Preservation or reconstruction of nasal valves

  • Evaluation and correction of inspiratory valve collapse

Recovery and evolution:

  • External and/or internal splints for 7-10 days

  • Significant edema for 2-3 weeks

  • Gradual refinement of the tip for 6-12 months

  • Final result established after 12-18 months

  • Possibility of revision rhinoplasty in 5-10% of cases, usually after 12 months
